Why we track this indicator
Regular well-child exams are preventative care that provides information on family stability, challenges, and/or issues the family faces. Providing care early in the child's life can promote positive socio-emotional adaptations that strengthen their over-all well-being. Additionally, an increase in connections between healthcare services and parents allows for better monitoring of the child's development.
WELL-CHILD EXAMS DATA BREAKDOWN
Status: Green
Definition: Well-child exams are routine medical visits that chart development and growth of the child. The percentage difference between total of the current quarter is compared to the previous quarter's percentage. No seasonal adjustments were made. The resulting percent difference is used to show a trend. This information is obtained through a collaborating partner.

Goal: The goal is to observe more well-child visits.
Traffic Light:
Green: 1% more well-child exams compared to previous quarter
Gray: within +/- 1% of previous quarter
Red: 1% fewer well-child exams compared to previous quarter

Indicators in Action
The ‘Data Action Mini Grant’ program will provide low barrier grants to collaborating partners which will help increase cross-system collaboration and facilitate the community response to needs and/or gaps identified through the Elkhart County Child Dashboard. Applicants may request up to $2,000. Two or more applicants can collaborate for collaborative action based upon one or more data indicators and increase the grant request. By leveraging the ‘Data Action Mini Grant’ program partners can have a platform that helps to inform community goals and creates a stronger sustainability platform for their programs.
