Parents receiving referrals through IN211 is an important indicator that sheds light on the overall community wellness of Elkhart County. Referrals for services is a key component in stabilizing families that helps improve health outcomes for children.

Community Resources Referred Data Breakdown

Status: Red

Definition: Number of monthly IN211 resource referrals per unique callers. Needs are considered met if the client obtains resources that address their inquiry to IN211. This indicator tracks the percent of met needs out of the total calls for services. This information is obtained through publicly available data.

Methodology

Methodology: IN211 reports the total calls it receives per month. In the report they identify the total number of calls and referrals made. Using this information we sum the total number of referrals made each quarter. This total is then compared to the previous quarter’s total.

Goal: To observe more referrals made compared to the previous quarter.

Traffic Light:

Green: at least 1% increase in referrals per quarter

Gray: within +/- 1% of previous quarter

Red: at least 1% decrease in referrals per quarter

Indicators in Action


The ‘Data Action Mini Grant’ program will provide low barrier grants to collaborating partners which will help increase cross-system collaboration and facilitate the community response to needs and/or gaps identified through the Elkhart County Child Dashboard. Applicants may request up to $2,000. Two or more applicants can collaborate for collaborative action based upon one or more data indicators and increase the grant request. By leveraging the ‘Data Action Mini Grant’ program partners can have a platform that helps to inform community goals and creates a stronger sustainability platform for their programs.
